The evil Gargamel has captured four smurfs so that he can have them for dinner. One of the smurfs must go out on a dangerous journey to rescue the others.
Gameplay
Mega Drive and Mega-CD versions
The game is a platformer, mostly played as Hefty Smurf, who must make his way to the exit of each stage. After defeating the first boss and rescuing another smurf, the next stage is played as that smurf. Each smurf has slightly different capabilities. Smurfs walk with and and run by holding . They duck with and jump with . The smurfs can defeat most enemies by jumping on top of them (with the exception of spiked enemies such as porcupines).
Smurfs start each life with three health points (shown as hearts) and lose one after taking damage from an enemy. The number of health points can be increased up to four by collecting health items. A smurf loses a life after losing all four health points, after falling into a bottomless pit, or after time runs out. The game ends after losing every life. The player is given a password after defeating each boss. There are three difficulty levels (Easy, Medium, and Hard).
Smurfs
Most stages are played as Hefty Smurf, but some stages are played as one of the other smurfs after they are rescued.
Hefty Smurf
Hefty Smurf can crawl with and . He can pick up items, such as springs, by pressing when in front of them and set them down with again.
Jokey Smurf
Jokey Smurf carries explosive presents, which he can throw with or set down with . The presents explode a short time after they touch the ground, which can be used to defeat enemies or to open sealed passageways. The explosion can harm Jokey if he is too close to it.
Brainy Smurf
Brainy Smurf carries a candle that illuminates dark caves.
Greedy Smurf
Greedy Smurf can throw cakes with .

Mushroom
Some items are found inside mushrooms, which can be opened by jumping on them.
Sarsaparilla Leaf
Collect 25 leaves to earn an extra life.
Star
Collect 25 stars to play a bonus stage.
Raspberry
Restores a health point, up to a maximum of 4.
Smurf Doll
Awards an extra life.
Key
Dropped by bosses when they are defeated. Pick up and use to open a cage containing a trapped smurf.

Hefty can enter the houses and appear on their roofs by pressing in front of a doorway.

The Raft Ride
Hefty rides down a river, collecting bonus items. The screen scrolls automatically, and the raft can be moved with and . The stage ends if the raft is trapped between an object in the water and the screen as it scrolls.
The Stork Journey
Hefty rides a stork through the sky in a mountainous region. The screen scrolls automatically, and the stork can be moved with and . The stage ends if the stork is trapped between the mountain and the screen as it scrolls.

Legacy
The 16-bit version of the game was re-released as Revenge of the Smurfs on the Game Boy Advance in 2002.
Versions
The Mega-CD version is identical to the Mega Drive version but has animated introduction and ending cutscenes and a CD audio soundtrack.
