Gameplay
The game is a platforming game where the player helps a boy penguin deliver an egg (containing a flower) to a girl penguin. The penguin does this by rolling the egg and dropping it down platforms until it reaches the bottom of the level. The penguin can break the ice blocks forming the platforms to open a path for the egg, but if the egg falls too far, it breaks. There are 30 rounds. The latest round and any previously completed round can be accessed directly from the title screen; the player's progress is lost after turning the console off, but passwords are given every five rounds.
The penguin moves with and and jumps with . He can push the egg in either direction by walking into it. The egg rolls out of the way if the penguin jumps on it. The penguin can break the platform in front of him with , breaking a hole in the ice that he and the egg can pass through. Levels scroll vertically, but the screen only scrolls when the egg is moved; if the egg is not moved to force the screen to scroll, the penguin can fall through the bottom of the screen and reappear at the top.
If the egg falls more than three blocks downward, it breaks. A blinking bar under the egg shows this distance. If the penguin lands on the egg but there is no room for it to roll left or right, it breaks. The round must be restarted if the egg breaks (and the game ends if the player runs out of eggs). The penguin is knocked out if touched by an enemy or hazard but recovers after a moment without needing to restart the round.
The game adds some assistance options, accessed through a menu after pausing the game with START :
- Map: Allows the player to pan the level with and . Pressing any button exits the map and returns to the game.
- Flag: Plants a flag that acts as a checkpoint where the penguin respawns if the egg breaks.
- Undo: Takes back the last move (such digging through a platform).
- Retry: Restarts the round (at the cost of an egg).

Music and sound
Much like Osomatsu-kun Hachamecha Gekijou, Ikasuze! Koi no Doki Doki Penguin Land MD features a notably loud and abrasive soundtrack. Sega CS programmed the game's soundtrack knowing it would likely be played back using low-grade television speakers, which would "filter" the music into a more pleasant sound while retaining the additional volume. However, with the advent of improved speaker technology, the game's soundtrack has gained a reputation for requiring additional audio filtering for modern play.
History
In Sega of Japan's official software listing, the game is simply titled Doki Doki Penguin Land (どきどきペンギンランド).
