One frame after the Grenade is launched.

When using the Grenade, Cluster Bomb or Banana Bomb, worms will bop the Grenade with their forehead to launch it, however the position at which a Grenade begins flying from is inconsistent with the throwing animation, being 7 pixels above the intended position regardless of which angle the Grenade is thrown at.[1]

This animation inconsistency is also shared with the IBM PC and PlayStation versions of the game, and was not present in the Amiga, Super NES and Mega Drive versions of the game.

Not fixed in any version.
