OK, new theory. Tatsuya Kouzaki is the sound assistant "Oyz" in Sonic the Hedgehog 2.
In the name Kouzaki, the following characters are present: こうざき
こ = ko こ = u ざ = za
Under the Polivanov cyrillization, こ equates to y, not u, even though the character "ゆ" is usually romanized as "yu" http://en.wiktionary.org/wiki/%E3%81%86 ("ko" and "za" are more or less the same)
So based on that terminology, his last name would be romanized as Koyzaki
