manual scanning isn't really my bag but I think we're trying to separate pages. There's a tool: http://forums.sonicretro.org/index.php?showtopic=30944 though you might need to ask for it since the link is dead. There'll be answers to the variable PDF page size thing too.
Also the file naming scheme for scans is "<name> <console> <region> <type>.whatever", e.g. "Bubsy MD US Manual.pdf" or words to that effect.
