James "Buster" Douglas Knockout Boxing has the same cast of characters as Final Blow, save for the removal of "Detroit Kid", who is predictably replaced with James "Buster" Douglas. Douglas is highlighted first in the West, while Japanese version starts with Dynamite Joe highlighted.
Douglas' weight in this game is the same as it was in the weigh-in against Mike Tyson in February 1990. "Reach" statistics are missing from the overseas version of the game.

As well as adjusting Detroit Kid's sprite to better resemble Buster Douglas, there are slight palette changes across the board. The background colours are dictated by the palettes of the boxers (this is true in both Final Blow and James "Buster" Douglas Knockout Boxing), however Douglas is the only boxer across the two versions to play in white (as he was when facing Tyson), meaning his matches are less colourful.
The Japanese release does not state the round number at the beginning of a fight.
