Model 2 version
The US version has a parental advisory warning, as issued by the AAMA.
Characters are measured in imperial in the US and metric everywhere else.
Honey is named "Candy" in the US, and her weight is proudly put on display at 90lbs. In Japan, this statistic is a "secret".
Picky is a walking advertisement for Pepsi in Japan. His Skateboard has the logo and his stage features a large Pepsi sign and articulated truck.
Saturn version
Most of the PAL game is presented at the full PAL screen resolution (in this case, 704x512, versus 704x448 for NTSC). The title screen is adjusted to compensate.
The pre-recorded matches in playback mode were translated for the international versions, and the Pepsiman demo removed.
In training mode, the commands were translated to English.
As with the arcade version, Honey became "Candy" overseas. Unlike the arcade version, her physical details weren't changed.
Picky's stage (and skateboard) has advertisments for Pepsi in the background in Japan, which were replaced with generic Sega logos in the West. The background was also rotated, so the objects appear in different positions relative to the fighting cage.
Finish the arcade mode with any character and the "Options Plus" menu becomes selectable. The US version however, lacks the "portrait" option (and non-Japanese versions lack the "Pepsiman reset" option, because they all lack Pepsiman).
Portrait changes
In addition to removing the "portrait" option, several of the images (which are now only shown during ending sequences) were replaced in the US version. The replacement images come from 22 renders from the introduction, usually visible in portrait mode:

Model 2 version
If used with a projection screen (such as that of the Super Megalo 2), an option exists which adjusts the game's colours, making them duller.
