Carls Cheat Screen
On the title screen press 
(CARL CARL). Start the game normally and you'll be taken to this screen.
changes the Level (between 1 and 6), 
toggles Bonus (0 for the normal level, 1 for the bonus section), and toggles Energy (0 for invulnerable, 1 for vulnerable).
"Carl" likely refers to programmer Carl Wade.
