Sega R&D #4 was made the fourth research and development lab within Sega, most likely started following a restructuring in 1986. The only reference to the department within a game is the credits for April 1987's Sukeban Deka II: Shoujo Tekkamen Densetsu.
According to Wikipedia.jp, Phantasy Star was also developed by R&D 4[1], which will need to be confirmed.
