Similar to the previous two games, Streets of Rage 3 is known as Bare Knuckle III in Japan. This time, more has been changed than just a logo - the Japanese version includes an introduction sequence and an animation where Axel punches the screen, which are completely missing overseas.

A recurring theme is that Bare Knuckle III uses a yellow highlight colour for certain pieces of text, while Streets of Rage 3 always sticks with silver.

The most striking difference in Streets of Rage 3 is that the palettes for Axel, Blaze and Skate/Sammy are different. In Japan, their attire is similar to Streets of Rage and Streets of Rage 2, while elsewhere they wear clothes similar to what was shown in the first Streets of Rage cover art.
The Japanese version of the game also sees characters measured in metric, while the overseas uses imperial measurements. The star system for describing abilities is replaced with yellow blocks in Streets of Rage 3.

Players will have to fight this character, Ash in Bare Knuckle III. Being a homosexual stereotype, he was removed from this part of Streets of Rage 3. In Japan, Ash drives a boat and then jumps out to fight, while elsewhere, stage 1 boss Sheeva drives the boat, speeding off to the right once some goons are dropped off.
