Version comparisons
Title screen
The SNES version has a publisher credit to DVS Electronic Co. while the Mega Drive version doesn't have a publisher credit.
Option
The Mega Drive version has the options set to Normal difficulty with three lives and continues by default and has no restrictions, while the SNES version is set to Easy and restricts the other settings according to the chosen difficulty.
Stage start
The SNES version only animates the top banner and the bottom banner is coloured blue. The Mega Drive version makes both banners move and the same colour, and the top banner replaces "NEXT" with the stage number.
Stages
In the Forest
All stages begin with the camera panning down from the top of the level and Pikachu falling down in the SNES version. The Mega Drive version has Pikachu on or close to the ground as soon as the stage begins. The HUD is also very different between both versions, with health and electrical power being displayed differently.
The Mega Drive version adds Pinsir as a mid-boss, and changes the level design to accommodate its inclusion.
It's Hot
The SNES version closes off one part of the map which would normally be open in the Mega Drive version and Mr. Nutz...
...redirecting the player into a hidden passage that is normally a lava pit.
The Mega Drive version makes the level's boss bigger and makes it stand up during its attacks.
Jump Around
The Mega Drive version takes away the boss' ability to jump onto the platforms, and allows Pikachu to attack it even when it's moving, making it easier to defeat. It is also slightly bigger in the Mega Drive version.
Underground
Stage 4 has a completely different appearance between the SNES and Mega Drive versions, with the SNES version copying graphics from Petrolith Castle from Skyblazer. The level designs are also completely different, with the Mega Drive version having a shorter, less difficult level.
Both versions feature Gengar as the boss of the stage, with Gengar being slightly bigger as well as being much easier to defeat as Pikachu can stomp it during any of its movements. In the SNES version, Pikachu can only jump on Gengar when it stops to flail its arms.
Hard Rock
Stage 5 also has a different appearance across both versions, with the SNES version copying graphics and some level design from Faltine's Woods from Skyblazer.
Both versions share the same graphics for the boss arena, however. While the Mega Drive version has a unique final boss in the form of Onix, the SNES version is instead a boss rush featuring palette-swapped versions of all the previous bosses with slightly different strategies.
Ending
The ending screen has scrolling text in the SNES version that congratulates the player. The Mega Drive version instead has a scrolling background.

The lizard enemy found in the first stage is copied and recoloured from Syd of Valis.
The dinosaur enemy first found in the first stage is copied and recoloured from an enemy in Stage 3-1 of Magical Taruruto-kun.
The dinosaur enemy first found in the first stage is copied and recoloured from an enemy in Stage 3-1 of Magical Taruruto-kun.
The bat enemy found in the final stage is another Magical Taruruto-kun enemy, also from Stage 3-1.

The foreground graphics are taken from stage 3-1 from Magical Taruruto-kun, and the background for this stage is edited from the Grassland and Forest stages from Tiny Toon Adventures: Buster's Hidden Treasure.
It's Hot
The foreground graphics and level design are taken from Volcano Underpass 2 from Mr. Nutz, and the background for this stage is edited from Zone A from Darius II.
Its boss is edited from an enemy that appears in Stage 3-5 of Magical Taruruto-kun.

The foreground graphics and level design are taken from Pete's Hideout from QuackShot Starring Donald Duck, and the background for this stage is edited from the factory stages from Donkey Kong Country.
Its boss is the Stage 1-2 boss from Syd of Valis but with a different palette.
Underground
The background and foreground graphics are taken from the forest area beyond Estafan Village from Monster World IV.
