The game is an update to Real Bout Garou Densetsu. It retains the basic mechanics and simplified control scheme of that game but updates many of the sprites, introduces new stages, removes the ring-outs, and returns to the two-lane fighting system of the earlier Fatal Fury games. It also adds back some characters last seen in Fatal Fury Special. The game is considered non-canon and has no storyline.
Characters move with and . They dash with or ; dashes can be canceled by tapping in the opposite direction. Characters crouch with and can move forward while crouched with (or when facing left). They jump upward with ; holding jumps higher than tapping . They jump behind and ahead with or . Characters can perform a punch with , a kick with , or a "Power Attack" with . Characters have different attacks depending on whether they are standing, crouching, or jumping. Pressing when far away from an opponent performs a taunt. Every fighter has special techniques performed by inputting specific directions with an attack button.
Characters can block attacks by holding the D-Pad in the opposite direction of the opponent. Blocking while standing guards against high attacks (from a standing or jumping opponent), while blocking while crouched defends against both high and low attacks. Blocking can also be done in midair. Throws that can be performed by holding the D-Pad toward the opponent and pressing the Power Attack button while in close range. Many characters have additional throws and grabs that use different button combinations.
Fights take place in two-lane battlegrounds. Players initially fight in the foreground lane but can switch into the opposite lane with . This can be used to dodge attacks or confuse the opponent. If the opponent is already in a different lane, the character can attack across the lanes, moving into the opposite lane in the process, by pressing the punch or kick button, or the character can knock the opponent into the same lane by pressing the Power Attack button.
The vitality gauge has two levels. It is initially colored yellow and fills with red as the fighter takes damage. After it is fully red, it starts to flash and fills with black as the fighter takes damage. The character is defeated when the vitality gauge is fully black. Each character has a Power Gauge that fills as the character performs normal or special techniques. When the gauge is at least half-full, it becomes colored yellow, indicating that the player can perform Break Shots, special counterattacks that can be performed after blocking an attack. When the Power Gauge reaches maximum level while the player still has more than half of the vitality gauge remaining, it enters "S. Power" level and begins to deplete gradually. During this time, the player can perform Break Shots or a Super Power moves, which are powerful moves similar to Desperation moves in the previous games. Using a Super Power move consumes any remaining energy in the Power Gauge. When the Power Gauge reaches maximum level while the player has less than half of the vitality gauge remaining (when the vitality gauge is flashing red), the Power Gauge enters "P. Power" level and gradually drains. In this state, the player can perform both Break Shots and Super Power moves indefinitely until the gauge runs out. The player can also perform a Hidden Power move, an even more powerful special ability (similar to the hidden Desperation moves in the previous game, which consumes the remaining Power Gauge. The contents of the Power Gauge are carried over between rounds but not matches.
Modes
The game has the following modes:
Arcade: The player selects any character and fights through a series of opponents. The player can choose any character as the first opponent, except for the four bosses. Billy Kane is always fought as the fourth opponent, and Laurence Blood and Wolfgang Krauser are fought as the eighth and ninth opponents respectively. A second player can enter the game at any time by pressing START to start a two-player match. If the player makes it to Wolfgang Krauser without losing a round and with a high enough score, the player can fight a "Dream Match" against Geese Howard (who died in the events of the last game).
Versus: The game has a dedicated two-player mode where players can choose any character and any stage to play.
Original: This mode plays the same as the Arcade Mode, except additional computer-controlled opponents sometimes challenge the player, similar to the experience of playing in an arcade.
Practice: A training mode where the player can choose any character to play and to fight against. The opponent is passive, does not attack or block, and cannot be defeated, and there is no time limit. The player's character has a permanent P. Power gauge and flashing health, so Super Power and Hidden Power moves can be performed repeatedly.

Geese Howard
Geese Howard is a crime boss who controls South Town. After obtaining the Jin Scrolls, he reconsolidates his power over South Town and seeks vengeance from the Bogard brothers. The player only has one chance to defeat Geese; the game cannot be continued if Geese wins.

Stages
Every character has a stage where matches are hosted, with stages shared by multiple characters. Every stage has recolored variants at later times of day for the second and third rounds.
Each stage has objects at the corners, which are destroyed if a fighter is attacked into them enough times. The character is stunned momentarily when this happens. Destroyed objects are restored after each round. Geese's stage is the only stage that does not have any corner objects.

History
Legacy
A sequel, Real Bout Fatal Fury 2: The Newcomers, was released in 1998. However, it was exclusive to the Neo Geo MVS and AES and Neo Geo CD and not ported to any other consoles at the time, so it is the only mainline 2D Fatal Fury game not to receive a port to a Sega console.
An update to the game called Real Bout Fatal Fury Special: Dominated Mind was released for the PlayStation exclusively in Japan in 1998. This version adds new moves and characters but removes the hidden characters and the two-line battle system.
